someone can explain to me why Hu Tao activate ghost special skill ?
Just looks like a visual bug. They occur occasionally.
Could it be the revive skill that he gets as an emblemed Fighter class hero?

no. he did revive, but AFTER revive he activate Anemona skill (ghost) for 4 rounds.

He has a wound ailment. He revived 4 times in a row and immediately died 3 of them.

no. he did died once. just once.
Are you sure? If he revives with one HP as shown and with the would ailment as shown he will immediately die again. If he revives again he will appear just as shown and repeat until the wound runs out or the revive fails. Unless he is ready to attack once revived you won’t have a chance to see him materialize before dying again.
