Ascend Levels?

Does anyone have a chart that shows the levels for 3*, 4* & 5* ascend levels? I mean what number level to go from one stripe up to the next…

I do not have an online spreadsheet, but here is a table with the values that you can use: all: I may have these values wrong, but fairly certain most are right. “NG” = “not good” in my shorthand.
